ACM Student Leadership Summit: Student Appointment Form (to be completed by staff member)

Please use this form to confirm the student you are appointing to the ACM Student Leadership Summit as outlined in the call for student appointments. Each ACM college will submit this form twice (i.e., one submission for each of the two students appointed by the college). Please speak with any student you appoint prior to submitting this form. After your submission of this form, the student you appoint will be contacted to confirm their participation. In order to secure the student's participation in the Summit, please ensure that you submit this form and the student submits the confirmation form no later than February 15.

You also agree that the details you provide will be utilized by ACM and Common Purpose to prepare for the program.
